Integrated Services operates 11 locality based teams that provide Assessment and Case Management across all client groups. We are committed in supporting our customers to leave acute hospital settings with timely and safe integrated discharge planning. Central Bedfordshire has created a number of positions based across the locality to work with our customers when admitted to the multiple hospitals that serve our Council area.

The service works in close partnership with other health and care systems and is currently reviewing the customer pathway to enhance experience and deliver good outcomes. You will have significant Health and/or Social Care experience, including multi disciplinary and inter agency working. Your ability to transfer your skills will help us achieve our goal in the delivery of excellent person centred approaches. You will value the impact of sound evidence based practice and understand that best practice results in best value. You will know the
importance of enabling people to be supported to be as independent as possible for as long as possible and that having a strong connection with their local communities can make a real difference to people’s lives and overall wellbeing.
